A container contains 40 litres of milk. From this container 4 litres of milk was taken out and replaced by water. This process was repeated further two times. How much milk is now contained by the container?

    I got your point but according to the question, the procedure is "take out 4 litres of MILK for the container and replace it with water"
    So, second time we repeat the procedure, we'll have to take 4 litres of MILK out, not the milk and water solution. That means we'll have to take (4 + 4/9) litres of the solution i.e. 4litres milk + 4/9 litres water.
    This way, the answer will be 28 only.
